Ive seen talk of billy having a mutated tapeworm, or like in diabolical, the brain tumour got powered up instead of billy.

Honestly it would be cool if billy ended up with supe killing powers. It would be awesome if he landed the killing blow to homelander.

it's probably a mix between his use of compound v and the brain tumour, considering his hallucination of becca controls it, a clear sign that something is wrong with his brain

it has been said in the past that using Compound V as an adult can lead to strange things happening as well (being a "freak" or "exploding") so yeah butcher probably has supe killing powers that aren't inherently powers
