[D4] How to get to those peeps?

I think the map shows where it is... I did not find a way to those guys. I searched for a hidden switch to open a passage, but could not find anything. Did anyone come across those guys or was this just a bug spawning mobs where there should be none?
Did you just click the alter of Lilith? I had them spawn in unreachable places a few times
I must have klicked it a few seconds before I spotted the guys on the left. But I can not tell for sure if they spawned when I clicked the altar or if they where already there.
