what are you supposed to fill at the first 'name'?
First name is the name everyone calls you by
Middle name is your second name
Last name is your family name

What do people enter in the middle name field if a someone's full name is more than 3 words long?
Hyphens or anything but your first and surname goes in middle
your last name / family name
