Get help diagnosing and treating a sick plant. Include pictures, watering schedule, light levels and temperature/location so that we can help figure out what's wrong.

About 3 months ago I got a nerve plant but it's not doing so hot. It's leaves are constantly turning brown and then black. It's also much less vibrant and lush then when I got it. I've always kept it watered so the soil is constantly moist just how nerve plants like it. I'm always watering it through the dish, not directly. I'm not sure if it's receiving enough light since it's on my window sill but do think it is as that spot seems to be lit up for most of the day. If anyone has some advice I'd be grateful :)

I'm not a plant expert, so I'm not an authority. I learned about flushing the soil from a roommate who'd just put the plant under the kitchen faucet for a minute or so, and let the water run through the pot and it the bottom. Since then, I've noticed that plants don't like hard water scale.
