You can't smoke indoors because people have asthma and even the lingering smell of smoke can trigger an attack.
Pretty sure you can drink in bars.
Yes, that's the exact point of the joke.
In the past, smoking and drinking at the same time was considered a normal activity, so much so that you could drink and smoke in a hospital.
Now, because smoking isn't allowed in bars, you can't drink and smoke in a bar.
