That reward is broken, it's a known error. Hopefully patch 1.1.1 will fix it.
Mike Ybarra said the goal for the fix was the 15th and Adam Fletcher more recently said it is still being worked on but didn’t give a timeframe. Sounds like it’s a tricky one.
The mystery tortured gift one seems to work (at least for me) but still no credit for the general one.
I finally managed to get a mystery chest and got the credit.
No input on the problem you're having, but this has saved me a lot of time trying to find mystery chests: Icy Veins Helltide Mystery Chest Locations.
This has been fantastic. I have used other maps, but this is by far the best. Since using this map I have been able to get a mystery chest every time. It even counted for my progress.
