It's made to look older than it actually is. Screw threads weren't really a thing until the 19th century. So this contraption isn't a medieval torture device.

Most outlandish torture things like iron maidens weren't really used but were 19th century people making up medieval torture fantasy stuff.

Good point. I always assume that these contraptions are pieces of art, and not actual historical artifacts. But this one is not presented as such.
