Especially if used in educational media, the 1 and 4 should be in a single column and different rows in order to maintain the method that's used in the first place, and produce a lower rate of accidental errors.

That is true, that would be better. And yet, without more context, I fail to recognize her attempt to be so far off that it's definitely, 100% obvious that she has made the egregious error that the meme implies.

Exactly. I've learned to write the '1' from the 16 below the 0 and then sum '4+0+1=5'.
