In my opinion, and personal experience, it also has a lot to do with not having enough time and energy to think about anything other then putting food on the table, or even where your next meal is coming from. Certainly there are people that just don't give a fuck, but I think that would be the minority if more people weren't living paycheque to paycheque.

IMO the term 'conservative' is somewhat fluid when the topic is other countries. For instance, Canadian/American conservatives/republicans (respectively) wouldn't even think of implementing a tax like that because in many ways they are the bankers.

Just to note, these people are well off. If it was someone that doesn't have $12,862 (or the equity/credit score to take out a large loan) to pay for surgery in another country (with a working medical system), then they'll die of being poor, and I'm sure many people have already.
