It's definitely not sloppy worldbuilding.
The character was born in the belt, but wants and pretends to be an Earther.
He even underwent surgery to look more like one. And the hat is part of it.
It would only make sense on earth, so by wearing it he signals that he's from there.
(Not sure if that is touched upon in the series, but it's explained in the books.)

What I don't remember is, does he just wear it like on earth, or does he use something to fix it in place? Even close to the surface "gravity" is lower, closer to the center any small jump and it might not stay in place.

They have artificial gravity in the belt at most stations created by the spin though not nearly as much as Earth's. Also Miller has never been to Earth.
