Florida's form linked by that article isn’t the same as OP’s. But yeah, that practice is fucked and point taken, OP’s could be from a high school in another state.

What country? I'd definitely give it more benefit of the doubt in Australia than in southeastern USA. COULD be an honest attempt to gather anonymous data that is going to be used in sensible ways.

It's not an honest attempt to gather anonymous data. There is n9 need for anyone yo ask these questions on a form other than for medical treatment or research where this info is directly relevant.

I mean, that has already been answered 12 hours before your comment. But if we really didn't know the context, it's not an unreasonable assumption that it's for research purposes; handing out questionnaires in school is by far the easiest (and thus cheapest) ways to gather data (anonymous or otherwise) on teenagers.
