Cut their throats. decapitation. or stab someplace other than the guts. stabbing into the back and upwards, aiming for the lungs works well. keeps them from making too much noise. (hey, I saw it on columbo.... it must work, right?)

It’s possible to do that encounter and get both the potion and Astarion’s love.

You have to convince him you actually love him and aren’t just using him the same way Cazador does.
